Registration
To register a child at Spring Lake Elementary, please bring the following items to the registrar from 8:00 a.m. - 3:30 p.m. Monday through Friday.
- Birth Certificate
- Social Security Card
- A Valid HRS 680 Florida certificate of Immunization - original required.
- Report Card - for grade verification if applicable
- Certificate showing a school-entry Health Examination performed within one year prior to enrollment (dated and signed by a health professional.
